Because asbl SuperNils considers the security of your data and your privacy extremely important, we ask you to read this page thoroughly first and to complete and return the form found further down this page.
During the experience within the SuperNils SuperVerse, you may leave certain data (a personal name, a hero name, you have conversations with peers in which you share certain things with each other, etc.). As this can be personal and sensitive information, we took someone under our wing who had specific knowledge about GDPR regulations. He helped us draft the content of this page correctly. This way, you can be sure that your data will be processed correctly and securely.
Although we do not take note of your personal data and do not store that data anywhere, there is still processing by vzw SuperNils within the meaning of the General Data Protection Regulation. That is why we still need to request the information below and obtain your consent before you can start with the wonderful project “SuperNils SuperVerse” – The World of AYA. We apologise for this administrative, but important, inconvenience. However, we will ensure that the burden is kept to a minimum.
The signed consent form, in which you agreement gives for the privacy policy/cookie policy from SuperNils non-profit organisation and for the processing of sensitive information and health data when using the virtual reality experience “SuperNils SuperVerse”, also applies to the following actions within this project:
- Using VR glasses to live the SuperNils SuperVerse experience
- Providing feedback on the project
- Providing feedback on the dance project separately
- Seeking peer contact through the VR SuperNils SuperVerse

Please also note that the board of SuperNils vzw and the partners who collaborated on this project have committed to treating all data we may receive (sensitive information, personal data and health data) as confidential. Information will never be shared with third parties without the prior consent of the person concerned.

2° Consent form for processing sensitive information and health data
On the right, you will find the link to the consent form for processing sensitive information and health data when using the virtual reality experience "SuperNils SuperVerse" - the World of AYA. You can download and print the document there, and then fill it in. The VR case itself usually contains a number of blank forms that you can use. If you are running out of forms, please notify the AYA team. They will ensure that new copies are provided.
After reading both the privacy policy and the consent form, you must fill in the missing details, date and sign the form. Then return it to the AYA team. They will store the document in a folder in a locked cabinet at the hospital.
PLEASE NOTE: you only need to submit this form the very first time you use the VR headset. Once the details have been filled in and signed, they remain valid for safe use of the application.
