As this can be personal and sensitive information, we took someone under our wing who had specific knowledge about GDPR regulations. He helped us draft the content of this page correctly. This way, you can be sure that your data will be processed correctly and securely.<\/p><p>Although we do not take note of your personal data and do not store that data anywhere, there is still processing by vzw SuperNils within the meaning of the General Data Protection Regulation. You can download and print the document there, and then fill it in. The VR case itself usually contains a number of blank forms that you can use. If you are running out of forms, please notify the AYA team. They will ensure that new copies are provided.<br>\n<br>After reading both the privacy policy and the consent form, you must fill in the missing details, date and sign the form. Then return it to the AYA team. They will store the document in a folder in a locked cabinet at the hospital. <br>PLEASE NOTE: you only need to submit this form the very first time you use the VR headset.
